Information we collect: name, contact details, medical and health history for the purpose of providing naturopathic services and contacting you in regards to these services. All your information is kept private and confidential and only used for relevant purposes with your consent. Health information is required to be kept for a minimum retention period of 10 years at which point we securely destroy it.

You have a right to ask for a copy of any personal information we hold about you and to ask for it to be corrected if you think it is incorrect. If you would like to ask for a copy of your information or have it corrected, please contact us at zoe@elevatewellbeing.co.nz
